National Provider Identifier (NPI) — a public identifier used solely to verify your practice entity. It is not Protected Health Information (PHI) under HIPAA.
A free-text description of your visibility challenges

Information collected automatically — when you visit this site, we and our third-party service providers automatically collect:

HIPAA Notice

Meridian Digital Strategy is a digital marketing agency, not a HIPAA Covered Entity or Business Associate in the context of this website. NPI numbers collected via our forms are public identifiers available in the federal NPPES registry and are used solely to verify practice entity information. We do not collect, store, or process Protected Health Information (PHI) as defined by HIPAA. We do not have access to patient records, clinical data, or health information of any kind. Please do not include patient information in any form submission on this site.

Children's Privacy
